Promoting the health and well being of all who live, work and play in Lake County.
The Be Well Lake County Diabetes program is a dynamic, patient centered intervention, designed to address the challenges to diabetes management. The Be Well program’s target population is the underserved throughout Lake County’s communities. The program provides integrated medical and dietitian visits, access to certified diabetes educators, group diabetes self-management classes, and community garden activities. The community garden is an opportunity for education, experience, and empowerment for patients to control and prevent diabetes.
These programs are in collaboration with Ike Hong and NorthShore University HealthSystem.
Plant donations are welcome at the beginning of the season for patients to learn and experience container and home gardening. Intermittent assistance is needed throughout the gardening year for maintenance. Donations of gardening supplies have proven to be helpful in the past.
