Live Well Lake County Eat Well Action Team

Organization Mission:

To empower those who live, work, and play in Lake County to have access, resources, skills and knowledge to eat healthy by cultivating strong collaborations and aligning community efforts.


The Eat Well Action Team is comprised of stakeholders throughout Lake County’s food system that have come together to identify interventions that will increase fresh food access in underserved communities.


Improve and promote nutritional opportunities in the food environment.

  • Objective 1: Increase the percentage of adults in Lake County who eat five or more servings per day of fruits and vegetables by 10% from 15% to 17% by 2021.
  • Objective 2: Increase the percentage of children in Lake County who eat two or more servings per day of fruits by 10% from 48% to 53% by 2021.
  • Objective 3: Increase the percentage of children in Lake County who eat three or more servings per day of vegetables by 40% from 20% to 28% by 2021.
  • Objective 4: Reduce the food insecurity rate in Lake County from 9.4% to 6.0% by 2021.


The team recently piloted Faithful Families, an evidence-based health education program for members of faith communities to make positive changes in their nutrition and physical activity. It was developed by the North Carolina State University and is now used nation-wide. The action team piloted the program in two faith communities in 2018, and is exploring more locations in which to pilot the program. The Action Team may have opportunities for people interested in learning more about selected interventions, such as Faithful Families. If you are interested in volunteering, please contact one of the co-chairs listed above.
